Pre-treatment instructions

  1. For your convenience you may download our registration form by clicking on the below link. After completing the form you may fax it to our office at 301-907-2935. By doing this you will save time by eliminating the need to do paperwork the day of your appointment.

  2. Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Occasionally, root canal therapy can be done the same day as the consultation. It is common in our practice to see emergency patients who present with discomfort or infection. When emergencies occur, we are able to accommodate patients as soon as possible, usually on the same day. However, a complex medical history or treatment plan will require an evaluation and separate visits for treatment.
  3. Please provide the following information at the time of your consultation:
    • Your referral slip and X-rays if applicable
    • A list of medications you are presently taking
    • If you have dental insurance, bring the necessary completed forms. We will provide you with the appropriate ADA codes, so you can file your claim as quickly as possible.
  4. Important: A parent or guardian must accompany all patients under the age of 18 at the consultation.
  5. Please alert the office if you have a medical condition that may be of concern prior to surgery (e.g. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (e.g. heart medication, aspirin, anticoagulant therapy, etc.)
  6. Pre treatment analgesia.
    The current studies in Dentistry indicate that a dose of non-steroidal anti-inflammatory medication prior to dental treatment can significantly reduce post-operative discomfort. As a benefit to our patients, an analgesic medication may be given prior to some treatment procedures. Use of analgesic medication immediately before your initial visit is discouraged, as it may affect the diagnostic tests we perform to determine the cause of your problem.
